GNYHA Webinar Addresses Needs of Justice-Involved People

GNYHA’s Social Determinants of Health Learning Series continued with a recent webinar on addressing the health and social needs of justice-involved people.

Shira Shavit, MD, Executive Director of the Transitions Clinic Network (TCN), reviewed the unique needs of the justice-involved population and discussed her organization’s primary care model, which serves the chronically ill with a history of incarceration. Dr. Shavit also highlighted research that associates TCN patients with reduced emergency department use and fewer episodes of preventable hospitalizations. TCN operates 39 programs across the nation, including at Montefiore Medical Center and SUNY Downstate Medical Center.

GNYHA shared tools and resources that member hospitals can use to educate staff about the justice-involved population’s needs. A webinar recording and related materials are available here. Registration is open for a follow-up webinar that will highlight NYC Health + Hospitals/Correctional Health Services programs, which provide medical, behavioral, and substance use services to people in the New York City jail system.
