GNYHA’s November 6 Symposium on Innovations in Patient Engagement will highlight statewide efforts to improve patient engagement in primary care. New York State Department of Health Delivery System Reform Incentive Payment (DSRIP) program Director Peggy Chan will provide an overview of DSRIP’s patient engagement project. The project requires participating organizations to use the Patient Activation Measure® (PAM) survey tool to find uninsured individuals or those who do not regularly use health care services and connect them to benefits and health care providers. Ms. Chan will discuss the project’s objectives and their importance to delivery system reform. Representatives from the Millennium Collaborative Care performing provider system (PPS) and Open Door Family Medical Centers, a federally qualified health center, will describe the PAM tool, workflows they implemented to identify patients who would benefit from its use, and ongoing coaching and specific engagement interventions. The panelists will also discuss new patient engagement initiatives that grew from the initial DSRIP project and how they will use PAM as a patient engagement tool moving forward.
Alan Manning, Executive Vice President of international patient-centered care organization Planetree, Inc., will deliver the keynote address. Representatives from NYC Health + Hospitals, Staten Island PPS, and Montefiore Health System will present their patient engagement strategies. Individuals responsible for patient experience, quality improvement, ambulatory care, and population health are encouraged to attend.
