The New York City Department of Health and Mental Hygiene (DOHMH) updated the deadline to October 30 for hospital enrollment in the COVID-19 vaccination program via New York City’s Citywide Immunization Registry. The revised deadline was announced in response to the accelerated timeline outlined by the Centers for Disease Control and Prevention.
Due to the shortened timeframe, DOHMH has advised the following:
- Complete Section A of the Provider Agreement for your Hospital/Network immediately and no later than Friday, October 30th.
- Prioritize completing one Section B of the Provider Agreement for each of your hospital locations by Friday, October 30th. Hospitals and Networks will be able to continue adding a Section B for each non-hospital vaccination location beyond the October 30th deadline.

Please note, the DOHMH October 30 deadline for COVID-19 vaccination program enrollment applies to hospitals in New York City or outside New York City with sites in New York City that need to be enrolled.
Hospitals Outside of New York City
For hospitals outside New York City or hospitals in New York City with sites that need to be enrolled outside the New York City area, the target deadline for enrollment is today, October 28. The New York State Department of Health (DOH) has indicated that hospital enrollment will not “close” at the end of the day. DOH understands that hospitals have been waiting for clarification on several questions to complete the applications.
Both DOH and DOHMH are encouraging hospitals to submit their applications as soon as possible for review to complete the enrollment process in advance of the first shipments of vaccine.
