DOH Survey to Vaccine Program-Enrolled Providers

The New York State Department of Health (DOH) has released a survey to all New York State providers who are enrolled or have pending applications with the COVID-19 Vaccination Program either with DOH or the New York City Department of Health and Mental Hygiene . Survey responses are due by 5:00 p.m. Monday, December 28.

The purpose of the survey is to collect information on facility/practice ability to assist in vaccination efforts as vaccine supplies increase over time and COVID-19 vaccine becomes available to additional priority groups. The survey is strictly a preliminary data collection exercise, and responses are for informational purposes only and are not binding for participation in any potential mass vaccination program.

The survey was sent to the individual designated as the primary vaccine coordinator on the enrollment application. DOH is requesting that each enrolled vaccination site complete a separate survey (please do not group responses across systems or multiple practices).
