The New York State Department of Health (DOH) will provide a new round of its Medicaid Accelerated eXchange (MAX) program, a rapid-cycle continuous improvement program developed under the Delivery System Reform Incentive Payment (DSRIP) program. The MAX program supports value-based payment arrangements by improving care for patients whose underlying unmet health care needs result in high utilization of hospitals and emergency departments. Previous MAX program participants for which complete data is available have shown an 18% reduction in readmissions six to nine months following completion of the program.
This round of the MAX program will include up to 24 interdisciplinary action teams, with a safety net organization leading each. Comprised of frontline staff from hospitals, primary care, skilled nursing facilities, home health, social service partners, and other organizations, the action teams will get workshop and coaching support over a five- to six-month period. The program also will include a “train the trainer” component to support future continuous improvement efforts. As with previous MAX programs, Amy Boutwell, MD, founder of Collaborative Healthcare Strategies, will lead. IPRO will provide additional workshop support.
Safety net providers and previous MAX participants can apply to participate in the new MAX program round. Interested participants are encouraged to participate in one of two DOH webinars.
